Supported Independent Living (SIL) and Accommodation/Tenancy Support
:
Personalised 24/7 assistance in shared or individual homes, with proactive sourcing/setup across Melbourne suburbs (e.g., Melton, Werribee, Cranbourne) and regional Victoria (Geelong, Bendigo, Ballarat), plus Western Australia options. Typical setups: 1–4 bedrooms (often en-suite), accessible features, and recreational amenities for fun/social engagement. Works in existing properties too. Learn more about SIL.

High Intensity Daily Personal Activities and Complex Needs Care: Specialised nursing-led support for complex medical needs (e.g., bowel/catheter care, tracheostomy/ventilator, wound management, seizure/diabetes care, enteral feeding, hoists/pressure care, medication/vital signs monitoring). 24/7 availability with emergency response and hospital transition coordination. Specialist Forensic Disability Supports: Tailored for justice-involved participants, including pre/post-release planning, reintegration/accommodation, CCO/Parole/Bail/STO/NCSO/DTO compliance, court/legal support, and community stability. Core Daily Living & Capacity Building Supports: Assistance with personal activities (hygiene, meals, routines), household tasks (cleaning/laundry/gardening), life skills development (communication/budgeting/problem-solving), community participation (art/sports/outings/events), employment access/maintenance, travel/transport, and community nursing (wound care/health monitoring).

Post-Hospital Transitions & Collaborative Care: Seamless discharge planning, rehabilitation, and ongoing support in partnership with hospitals, GPs, allied health (OTs/physios/speech), and behaviour practitioners.
