Initial Assessment
We review the patient's NDIS plan, medical needs, and care requirements. Our team can meet at the hospital if needed.
Accomodation Coordination
We support participants in finding accommodation that meets their needs, as well as helping to organise staffing and create detailed care plans. We collaborate with support coordinators and the full care team.
Seamless Transition
We assist in creating a seamless transition to ensure discharge, transport, medication handover, and all other relevant documentation and procedures are adhered to.
